3Rs Admission Requirements

3R’s Child Development Center provides education and childcare services to children without regard to race, creed, religion or economic level. Therefore, any child between the ages of 6 weeks to 12 years is eligible to apply for enrollment. The enrollment and admission procedure includes your completion of the required registration forms. The registration fee and first week’s tuition are due on your child’s first day of attendance. Please complete all forms in your enrollment packet.


Please advise the Center Director of any changes to your address, home telephone number, work telephone numbers, emergency numbers, place of employment and persons authorized to pick up your child(ren) by completing an update form.

A health summary is due at the time of admission. This must include an updated immunization record. Children who do not have a health summary or immunization record cannot remain in the center. Children in our Infant, Toddler, and Preschool programs must have a new health summary each time they move to a new age group.

General Information

Any child between the ages of 6 weeks to 12 years is eligible to enroll.

Medical Requirements

Your child must have proof of a physical exam, including immunizations, upon admission to the center. All immunizations must be up-to date at the time of enrollment. A copy of your child’s physical exam and health records will be kept on file and reviewed periodically. It is your responsibility to notify the center when your child receives new immunizations and to furnish proof of them to the center.

A current immunization record must remain on file at the center for continued enrollment. If an immunization is not given in a timely manner or cannot be given, a note from the physician will be required to remain in the program. An immunization schedule is available from the Executive Director.

Daily Communication

You will be provided with a daily schedule of activities your child will experience. Field trips, special events and optional enrichment programs are posted on the Parent Bulletin Board. We encourage you to review it daily.

Parent-Teacher Conferences and Visitations

In addition to pre-admission, parents have an opportunity to meet with their child’s teacher at a scheduled parent-teacher conference. We encourage you to schedule any additional conferences you desire. We will do a written assessment for each parent to review at conference time. We will have two conferences per year.

3R’s has an open door policy. Parents are welcome to visit the center anytime during the hours of operation.
